What is Smile Design?

An aesthetic smile; reflects many positive values such as health, success, strength and beauty. Smile design is the applications made to obtain a natural and aesthetic appearance of the teeth and gums, whose appearance is deteriorated for any reason, by considering both the face shapes and the needs of the patient. Smile design is of great importance for people to have a beautiful smile and regain their lost self-confidence. Factors such as face shape, skin color, gender, age, lips, alignment of teeth, gingival form, mouth and jaw structure are taken into account in smile design. The aim of the treatment is to give you the most natural and aesthetic smile.

What Treatments Does Smile Design Include?

Since each person's smile design needs are different, the procedures to be performed will vary accordingly. According to the person's request and need, the following procedures can be applied in smile aesthetics. In some patients, only one of these treatments is applied, while in some patients more than one procedure may be required to achieve the desired result. These processes can be exemplified as follows.

  • Porcelain veneers (crowns),

  • Zirconium coating,

  • Laminate veneer (leaf porcelain),

  • Gum aesthetics (pink aesthetics),

  • Teeth whitening (bleaching) if the color of the teeth is dark,

  • Teeth cleaning, if necessary, weighing or curettage,

How to Smile Design?

The first step of smile design is to correctly understand the aesthetic expectations and demands of the patient. Because the procedures to be applied during the aesthetic smile design are determined according to these demands and needs. Because everyone's facial structure, smile, amount of gum appearance and aesthetic perception are different. That's why smile designs are made individually.

First, the patient's mouth size is taken and the models of the teeth are obtained. On these models, the patient's problems are determined in detail and corrections are made on the model. The patient can see the finished shape of the teeth on the model. Then, this model is transferred to the mouth and the patient has the chance to experience in his own mouth what the teeth will look like after the treatment before the procedure is started. If there is a part that he wants or does not like to be corrected, it is determined and corrected at this stage and the patient does not encounter any surprises after the treatment.

Another form of design is digital smile design. In this design, the intraoral and facial photographs of the patient are made compatible with the face shape by making digital measurements in the computer environment. The most suitable images for the appearance that will emerge after the treatment are also obtained at this stage. The physician tells the patient what kind of procedures should be done to the existing teeth in order to achieve the ideal smile design, together with the alternatives, if any. As a result of these studies, the physician starts the treatment according to the accepted treatment procedure. All treatments and the current situation are recorded and everything is planned according to digital design.

What is Wax-up and Mock-up in Smile Design?

This model of the design made with wax in preparation for the smile design is called Wax-up. Afterwards, the model produced from silicone is called Mock-up, to be applied to the teeth over the Wax-up model. Wax-up and Mock-up stages are very important when designing a smile. Wax-up and Mock-up application offers us the opportunity to predict the result of the treatment at the very beginning of the treatment. This gives us a lot of information such as how much change we will make in the teeth at the beginning of the treatment, how much we will wear from which tooth, and how the teeth will look in the final; It offers the opportunity to experience the finished prosthesis in their own mouth without encountering any surprises after the treatment.  Thanks to this application, the preliminary design of the restorations to be made on the teeth is created. Another reason why the wax-up and mock-up process is important is that it allows us to make the most ideal smile design for the patient by making changes in the prosthesis according to the expectations of the patient who sees the preliminary design.

How to Wax-up and Mock-up?

In the first session, digital or manual measurements are taken from the patient. On these measurements, the smile design, which is planned to be made from wax, is shaped in the laboratory on the model, after the digital measurement, with three-dimensional printers, and after the manual measurement. The processes done so far are the Wax-up part. In the mock-up phase, the design is applied to the teeth by means of a silicone key prepared by the laboratory on the wax-up model. The patient and the physician evaluate the smile design in the mouth, note the shape, size, color or form that is desired to be changed and send it to the laboratory.

What is Digital Smile Design?

Digital smile design is the creation of a personalized, aesthetic smile design by dentists using digital technologies. In the field of dentistry, digital design software is used in our country as well as all over the world. In these software, the tooth structure and facial proportions of the person are analyzed in depth. Thanks to digital software, dentists can achieve the most accurate harmony between the patient's tooth structure, gums, lips and face. Thus, a natural and aesthetic smile can be created for the person.  In digital smile design, a patient-oriented treatment is performed.  The smile style that suits each person is different. That's why dentists design the most suitable smile for the person.​

In digital smile design, it will take two sessions to make the design and show it in the mouth.  Dentists take the patient's records in the first session. Then, in the second session, the smile design made for the patient on digital software is shown. If the patient approves the design made after the digital smile design program, the necessary studies begin immediately. If revisions need to be made, digital smile design can take up to three sessions.

How to Make a Digital Smile Design?

The first appointment in digital smile design goes like this;

  • A digital impression of the teeth is taken using a 3D intraoral scanner.

  • Photographs of the patient are taken from different angles in a private studio.

  • Case examples are shown to the patient in the form of a before/after made by the dentist.

  • Thanks to special software, digital smile design is made.

The second appointment in digital smile design is now the process where the processes begin. The second appointment goes like this:

  • Necessary procedures are performed for the patient's teeth and gums.

  • Thanks to the 3D intraoral scanner, digital measurements of the teeth are taken.

  • The patient's approval for digital smile design is obtained and necessary revisions are made.

How long is the treatment period in smile design?

The duration of treatment in smile design varies from patient to patient. The expectation of the patients and the oral structure generally determine the duration of the treatment.  This period can sometimes be a single day, and sometimes it can vary between 2 and 15 days. Therefore, clear information in the treatment process can only be learned after the examination of the specialist physician.

Smile design prices vary according to the patient's expectation, the type of material used (zirconium, Emax), the method, the patient's current tooth structure, and the amount of procedure to be performed. Sometimes only the treatment with aesthetic filling is sufficient, sometimes the position of the teeth is sorted with orthodontic treatment (with wires or clear plates) and a complicated treatment with whitening leaf porcelains may be required. Therefore, clear information about prices can only be learned after the examination of the specialist physician.

Treatments Applied in Smile Design
Pink Aesthetics

It is the process of regaining the shape, form and color of the gums, which has an important place in pink aesthetic smile design. To have an aesthetic appearance and smile, the gums, teeth, lips and chin should be in harmony. Healthy gums have a pink, slightly rough surface.  Before the pink aesthetic treatment, gum diseases such as gingival recession, gingival swelling, gingival bleeding and redness, gingival sensitivity should be treated.  Different treatment methods are applied for gingival aesthetics, depending on the presence of the problem: In patients who complain of the appearance of the gums when they smile, they are reshaped and leveled by cutting the gums with laser or surgically. Even with small corrections made with pink aesthetics, great results can be obtained in most cases.

Leaf Porcelain (Laminate Veneer)

Laminate Veneer (leaf porcelain), which is one of the first preferred treatments among smile design treatments, is a type of veneer with a thickness of 0.3-0.8 mm, which is applied to the teeth without abrading the teeth or with minimal abrasion only on the front surfaces.

Full Crown Restoration (veneer)

Full crown restoration Unlike Laminate Veneer (leaf porcelain), the application is made to the entire tooth. It is preferred in teeth with excessive substance loss. Smile design applications are one of the most preferred treatment methods after Laminate Veneer (leaf porcelain). While full ceramics (Emax, Empress) or zirconium coating are frequently used in smile design, metal-supported porcelain coatings are not preferred today because they cannot meet aesthetic requirements.

Composite Lamina, Aesthetic Filling (Bonding)

Composite laminate, without damaging the teeth; These are aesthetic and cosmetic restorations made with composite material on tooth surfaces in order to change the color, shape, enlarge or treat a defect and give the tooth an aesthetic appearance. This dental aesthetic treatment, also known as bonding; It is an inexpensive but effective way to treat damage, cracks and fractures on teeth compared to leaf teeth. The advantages are that less tooth material is usually removed. It has fewer clinic visits and can sometimes be prepared in a single session.

Orthodontics is the science that examines the crookedness of the teeth, the relationship of the jawbones with each other and the skull bones, and diagnoses the existing problems related to them. Orthodontic treatment is a way of treating the disorders in this system. In some cases, Smile Design can be completed with only orthodontic treatment. Aesthetics can only be treated with orthodontics in teeth with position defect and no deformity. In some cases, it is used to straighten the position of the teeth before Laminate Veneer (leaf porcelain). The advantage of this process is that less material is lost from the teeth.
